WHAT CAN I EXPECT?
After you have scheduled an appointment with us, we will reach out to you via email with a link to our online intake forms. Please complete the intake form prior to your appointment (preferably at least 24 hours, so Dr. Underwood has time to review your health history). *The forms need to be completed using a computer or tablet; they are not compatible with cell phones.
On the day of your appointment please wear comfortable clothing and bring your training shoes. Please arrive right at the time of your appointment. Dr. Underwood will review your health history and nature of your injury/condition with you and perform a physical examination and analysis of your mobility and stability. He will discuss his findings with you and together you’ll decide on a course of care consistent with your goals. Dr. Underwood will then provide initial treatment. If he determines that you need to see a different health care specialist or if he feels you need imaging or other diagnostic testing, he will provide you with specific recommendations.
